Dear parents and students,

Our class, is going on a Field Trip. This form is to notify you of the trip. 
Please sign on the line below if permission is granted for your child to go. 
Then have your child return the bottom half to Mr. Cap .
 Keep top half for field trip information.

Name of field trip:                              6th Grade to Woolaroc Museum

Directions: To reach Woolaroc from Tulsa, OK on US 75:
• Take US Highway 75 north from Tulsa, Oklahoma
• Exit at OK State Highway 20 and proceed left (west) four miles to Skiatook, OK
•Turn right (north) at the first traffic light onto OK State Highway 11
• Proceed north or west on OK State Highway 11 to OK State Highway 123
• Turn right on OK State Highway 123
• Proceed eight (8) miles to the Woolaroc entrance on the left

Place:                                                   Woolaroc Museum

Date:                                                    Wednesday November 9th

Time Leaving and Returning:                 8:30am  to 3:10pm

Dress Code:                         Spirit Day Dress, (jeans and Grace or Christian tee shirt)
                            It might be chilly, bring jacket for lunch outdoors and walking trails.

Cost :                                                    5$ for bus and  money for lunch or sack lunch
