Dear parents and students,

Our class, is going on a Field Trip. This form is to notify you of the trip. 
Please sign on the line below if permission is granted for your child to go. 
Then have your child return the bottom half toMr. Cap .
 Keep top half for field trip information.

Place:                                                   Gilcrease Art Museum

Date:                                                    Wednesday September 20th

Time Leaving and Returning:             10:00am  to 2:05pm  10:40 docent-guided tour of American West.
                                                                                      Then docent- guided tour of Audubon around 11:30

Dress Code:                                         Prep casual with belts, shirts tucked at all times.

Cost :                                            3$  for bus fee and  money for lunch @ Don Pueblo or sack lunch
