Our class, is going on a Field Trip. This form is to notify you of the trip. 
Please sign on the line below if permission is granted for your child to go. 
Then have your child return the bottom half toMr. Cap .
 Keep top half for field trip information.

Time Leaving and Returning:       10:10am to ORU till 12:05 then Wendys for lunch then to GCS @1:30pm

Dress Code:                                         Chapel clothes w/o the jackets

Cost :                                                    3$  for bus and  money for brunch
