Dear parents and students,

Our class is going on a Field Trip. This form is to notify you of the trip. 
Please sign on the line below if permission is granted for your child to go. 
Then have your child return the bottom half to Mr. Cap.
 Keep top half for field trip information.

Name of field trip:           US History Class to Gilcrease Art Museum and -

Place:                                                   Gilcrease Art Museum

Date:                                              Thursday September 28th

Time Leaving and Returning:             10:00am to 3:00pm 10:40 docent-guided tour of American West.
                                                                    Then docent- guided tour of Audubon around 11:30. Go to mall
for Food Court lunch @ 1:00. Then heat to Washington Irving Park
@ 1:45 -144^th and Memorial.

Dress Code:                                         Chapel dress with out jackets

Cost :              3$ for bus fee and  money for lunch @ Mall or sack lunch
